As of November 1, 2021, Trinity Medical Group  will be contracted directly with UnitedHealthcare and will no longer work through the intermediary company WellMed. Our new arrangement with UnitedHealthcare will not change your insurance, clinician, clinical team, care management, or future appointments.

In the coming weeks and months please look out for new benefits cards from UnitedHealthcare for plan years 2021 and 2022.

  1. What is happening?

By working directly with UnitedHealthcare, Trinity Medical Group will improve coordination of your care, access to UnitedHealthcare resources, and your ability to manage your health and wellness.

Our new arrangement with UnitedHealthcare will not change your insurance, clinician, clinical team, care management, or future appointments. In the coming weeks and months please look out for new benefit cards from UnitedHealthcare for plan years 2021 and 2022.

  1. Why am I receiving two new benefits cards?

The first card you receive will replace your benefits card that has the WellMed logo on it and will be your benefits card for the remainder of plan year 2021. You will receive another card before the end of the year that will serve as your benefits card for all of 2022.

  1. What do I need to do?

Once you receive your new UnitedHealthcare benefits card, you should safely dispose of your old benefits card.
