How do I know if Trinity Medical Group physicians participate in my health plan?

To receive full insurance benefits, insurance providers require patients to receive services with in-network facilities and physicians. To confirm requirements and benefits, call your insurance provider.

What is the cancellation policy?

Please call us at least 24 hours before your appointment if you need to cancel or reschedule for any reason.


What are your hours?

Our regular office hours are as follows:

  • Lakeland Office: Monday–Friday 8am–5pm
  • Winter Haven Office: Monday–Friday 8am–5pm

What should I bring to my initial appointment?

You should bring copies of all pertinent medical records, lab results, and imaging results to help your physician understand your medical history as well as diagnose and offer treatment options. You should also bring your insurance card, referral, and pre-certification (if applicable).


How early should I arrive before my appointment? What should I expect?

When you schedule your appointment, our staff will inform you when you should arrive for your visit, what you should expect, and how you should prepare for it. To expedite your registration, please print and complete our forms and bring them with you. For initial appointments, we recommend that you arrive at least 15 minutes before your scheduled appointment time to complete any remaining paperwork.


I am traveling out of the country and need a vaccine. Can I come in for an appointment beforehand?

Yes. We offer comprehensive travel-related services including physicals, vaccinations, and prescriptions as appropriate. Your physician can also answer any questions you might have about maintaining your health while traveling overseas.


How can I get a copy of my test results and medical record?

To receive a copy of your medical records or test results, please call:

You may be required to sign a consent form for release of medical information if records are to be sent to another hospital or office.


How will I know how much I owe?

Your insurance provider will send you an Explanation of Benefits that details the amount it has paid, any non-covered or denied amounts, and the remaining balance that you are responsible for paying to Trinity Medical Group. Many health plans require patients to pay a co-payment or deductible amount. These balances will be collected at the time of registration. Review your Explanation of Benefits carefully, compare it to your billing statement, and call your insurance provider right away with any questions or concerns.


What forms of payment do you accept?

For your convenience, we accept cash, personal checks, Visa, MasterCard, Discover, and American Express. We will charge your credit card only for the amount you authorize.
